Filling in and submitting medical reports in different situations – instructions for physicians

Traficom prepares instructions and forms for physicians on the assessment of fitness to drive road transport vehicles. Traficom does not have competence to rule on assessments physicians make regarding individual applicants’ fitness to drive.

Traficom is responsible for issuing driving licences and other personal licences for road transport. Traficom receives applications via its service provider Ajovarma's service points. Customers can also submit driving licence applications via electronic service channels, if they meet the preconditions of using e-services.

If a medical report concerns an application for a driving licence (first driving licence, upgrading a driving licence category or renewing a driving licence), always give the report to the customer (form F122). Do not send the medical report to Traficom!

A medical report must always state whether the customer meets the requirements for fitness to drive. If a physician is unable to issue a statement on the customer’s fitness to drive, he or she should not issue the medical report at all.

The police are responsible for supervising drivers' fitness to drive and making decisions on related measures, such as driving bans. Medical reports related to the supervision of fitness to drive are submitted to the police.

A medical report is not required for driving licence applications in the following situations:

Group 1 (moped, light quadri-mobile, tractor, motorcycle or passenger car)

  • applications for a first driving licence if the applicant can give a self-declaration assuring that he or she meets the medical standards of fitness to drive
  • applications for a new right to drive a Group 1 vehicle if the applicant already has a valid right to drive a Group 1 vehicle
  • applications for the renewal of a valid driving licence, if the application is submitted at the latest on the day when the applicant turns 70

Group 2 (lorry, bus and coach)

  • applications for a new driving licence category if the applicant already has a valid right to drive a Group 2 vehicle
  • applications for the renewal of a valid right to drive a Group 2 vehicle if the applicant is under the age of 45 and his or her current driving licence expires before the applicant turns 45

A medical report is required in the following situations:

Group 1 (moped, light quadri-mobile, tractor, motorcycle or passenger car)

  • applications for a first driving licence if the applicant cannot give a self-declaration assuring that he or she meets the medical standards of fitness to drive. NB: The medical report must be submitted using the form F122 (medical report on fitness to drive). Read more about the self-declaration by first-time driving licence applicants.
  • renewal of a driving licence if the applicant’s licence has already expired
  • renewal of a driving licence if the applicant is over the age of 70. In this case, an extended medical examination is required.

Group 2 (lorry, bus and coach)

  • first-time applications for a right to drive a Group 2 vehicle
  • renewal of a driving licence if the applicant’s licence has already expired
  • renewal of a current driving licence, if the applicant has turned 45 or will turn 45 during the validity of the current licence. If the applicant has turned 68 before renewing his or her driving licence or will turn 68 before his or her current licence expires, an extended medical examination is required.

NB: Do not send the medical report to Traficom. Medical reports prepared for applications should never be sent to Traficom or the police. If the medical report (form F122) concerns an application for a driving licence or the renewal of a driving licence (right to drive has expired or is about to expire), give the report to the customer or enter it into the Patient Data Repository in the Kanta Service.

 

The Patient Data Repository is one of the Kanta Services. Healthcare organisations can save patient data in the repository from their own information systems in a secure manner. If the form F122 on a medical report on fitness to drive (in Finnish ‘Lääkärinlausunto ajokyvystä’) has been saved in the Patient Data Repository in structured format, Traficom can receive the conclusions of the report in electronic format for a driving licence permit. In this case, the information can be accessed in the Patient Data Repository via the service Delivery of Medical Certificates. Medical reports issued for other application types or other purposes cannot be stored in a format directly accessible to Traficom.

If the information on fitness to drive is only entered in patient data, Traficom is unable to access it electronically during the process of issuing a driving licence. To enable Traficom to use the information electronically, the form F122 on a medical report on fitness to drive must be entered in the Kanta Services using a form app or other technical means. In other words, the healthcare patient data system must have the technical capacity for entering the data in the correct form.

Physicians should send medical reports to the police in the following situations:

  • to maintain valid a Group 2 driving licence issued before 19 January 2013 when the driver turns 50, 55, 60 or 65 years (driving licence remains valid until the driver turns 70)
  • if a physician has required the applicant to submit a medical report earlier than usual to ensure that the medical standards for fitness to drive are met
  • notification to the police on changes in fitness to drive (F203)
  • medical reports required by order of the police.

Do not send the medical report to Traficom!

Medical reports concerning applications for a parking card for people with reduced mobility must always be given to the customer.

Do not send the medical report to Traficom or the police.

The report must be issued with the relevant Traficom form (Lääkärinlausunto liikkumisesteisen pysäköintitunnusta varten, available in Finnish and Swedish), in which the physician gives an unambiguous statement on:

  • whether the customer meets the requirements of the parking card and
  • whether the condition or disability is permanent or temporary.

There are two ways for customers to apply for a parking card for people with reduced mobility:

  • by using the Traficom e-Services
    • The customer adds the medical report as an attachment to the application in the service.

or

  • by visiting one of the service points of Traficom’s service provider Ajovarma
    • The customer submits the medical report as an attachment to the application.

Regulations

The purpose of this guideline is to support healthcare professionals in assessing a driver’s fitness to drive in different situations. The guideline is based on legislation on fitness to drive, and it was drawn up by Traficom together with experts of clinical specialities. However, this document is not a diagnostic guideline or a handbook intended for specialities. Most driver health assessments are indeed carried out in primary health care and occupational health care services. The guideline sets out criteria for the clinical assessment of fitness to drive in healthcare services and provides grounds for discussing issues related to fitness to drive with the individual and, if necessary, also justifications for a potential driving ban.
