Incident and Occurrence report
File a report, if you have experienced an incident during drone flight or noticed any events, which may cause a flight safety risk.
Regardless of the operational category of unmanned aviation (open, specific, certified), the remote pilot of an unmanned aircraft must always submit an aviation safety report if:
a) the unmanned aircraft has been involved in an occurrence that has resulted in a fatality or serious injury to a person, or
b) a manned aircraft has been involved in the occurrence.
In addition, you may always submit a voluntary report for situations other than those mentioned above if you observe events that, in your opinion, constitute an actual or potential risk to aviation safety.
Organizations are required to submit reports using the ECCAIRS2 system.
If you are a private individual, you may submit a report using the aviation safety report for private pilots -form.
Instructions for completing the forms can be found on the forms themselves. Here are some general guidelines:
- Fill in the reporter’s details
- Provide details of the unmanned aircraft. More detailed information about the make and type of the aircraft can be included in the event description section
- On the private pilots reporting form, the aircraft registration field does not recognize an unmanned operator registration number, so you may enter “OH-XXX” there. Provide the operator registration number in the event description section
- If another aircraft was involved in the occurrence, provide its details in the event description as accurately as possible: manned/unmanned, fixed-wing or rotary-wing, aircraft size category, registration
- Write as detailed a description of the occurrence as possible, including your assessment of what caused it. When completing the form, spend most of your time on the event description section
- Select the most appropriate option from the occurrence category classification
- Then select a more specific event type in the classification details
In accordance with the aviation Just Culture principles, submitting a report will not result in consequences for the reporter, provided the occurrence was not caused by gross negligence or intentional misconduct.
